The following describes how we may use and share your health information. For treatment, payment, and healthcare operations, we do not need your further authorization.

  • Treatment — sharing information with other providers involved in your care, including dermatopathology labs and referring physicians

  • Healthcare operations — quality assessment, staff training, scheduling, and administration
  • Appointment reminders and information about treatment alternatives or other health-related benefits
  • Disclosures to family members, friends, or others you identify as involved in your care, with your permission
  • Business associates who perform services on our behalf (such as billing, scheduling, or practice management) under confidentiality and Business Associate Agreements

Less common uses (as permitted or required by law):

VIII. Text Message (SMS) Communications

If you check the text-message box on our appointment or booking forms, you are giving express written consent for Sergay Dermatology to send you text messages at the mobile number you provided. Those messages cover your appointment request, appointment reminders and scheduling changes, and replies from our team.

Consent is entirely optional and is never a condition of receiving care, scheduling an appointment, or any purchase. Message frequency varies. Message and data rates may apply. Reply STOP at any time to stop receiving texts, or HELP for assistance. You may also ask our office to remove you at any time.

We do not share or sell mobile information. Mobile phone numbers collected for text messaging, and consent to receive text messages, are never shared with or sold to third parties or affiliates for marketing or promotional purposes. Numbers are shared only with the messaging service providers that deliver the texts on our behalf, and only for that purpose.

IX. Website Information, Cookies & Advertising

When you visit sergaydermatology.com, we also collect limited website information — such as your IP address, browser and device type, and how you interact with the site — in order to operate and improve it. We use cookies along with analytics and advertising measurement tools (including Google Analytics, Google, and Meta) to understand how visitors find and use the site and to measure the performance of our advertising. These tools may receive limited technical information such as your IP address, device type, and the pages you view.

You can control cookies through your browser settings, and manage ad personalization through your Google and Meta ad settings.

We do not sell this website information, and we never share it with third parties so they can market their own products or services to you.
